We will be glad to assist you in checking if you have chiropractic benefits under your policy as well as the terms and conditions. The information we will need from you to inquire on your behalf are the following:

-First and Last Name

-Copy of your insurance card (front and back)

-DOB (Date of birth)

For any other insurance company, we will provide you with the necessary forms for reimbursement.

We accept most of Auto & Work Injury Insurance.

We don't believe in itemizing your health care, so we've made our fee schedule as simple and affordable as possible. For an initial visit, consultation and adjustment (assuming no further diagnostics or imaging is necessary to proceed) the cost is $100. For follow up visits that include an adjustment alone the cost is $60.
